Role summary
This role is for a Band 6 Registered Nurse to join a dynamic Community Nursing Team, providing high-standard, evidence-based holistic care to housebound patients in their homes to prevent unnecessary hospital admissions. The successful candidate will manage a caseload, focus on public health and preventative care, and support the team in implementing and evaluating care programmes. They will also clinically supervise and mentor junior staff, contribute to service improvement, and manage budgets.
Essential criteria
- Registered Nurse (1st Level)
- First level degree in related subject
- Evidence of Continuous professional Development relevant to the field of practice
- Evidence of leadership or management training
- Practice Educator and Assessor Preparation (PEAP) or equivalent qualification
Desirable criteria
- District Nursing Specialist Practitioner Qualification or working towards it
- Coaching CPD
- Significant Post registration community experience
- Post registration experience of caseload management
- Experience of Personalised Care Planning and complex patient management
- Ability to take appropriate action to assess and manage risk
- Experience of empowering a positive learning culture for interdisciplinary and interagency teams
- Proven experience of working autonomously in challenging situations
- Experience in managing complaints
- Experience in service improvement
- Understanding of clinical governance, audit, and risk management.
- Advanced clinical decision-making skills to support patient centred care
- Advanced clinical skills to include a variety of patient conditions and comorbidities
- In-depth understanding of community nursing and long-term condition management.

We have an exciting opportunity for Band 6 Registered Nurse to join our dynamic, forward thinking Community Nursing Team.
Our aim is to provide a high standard of evidence based holistic care to housebound patients within their own homes so as to avoid unnecessary hospital admissions.
The successful candidate will have a general knowledge of current best practice in Community Nursing. Ideally would have health assessment or has a confirmed start date to the Programme. Be an effective leader with excellent communication skills and have the ability to manage a caseload of patients.
We will provide you with a structured induction and training period to ensure you have the opportunity to develop skills and knowledge. We encourage staff and support continuing professional development progression.
The Service operates seven days a week and flexible working is required.
