Role summary
This role is for an experienced and motivated registered nurse to join the Warrington Out of Hours District Nursing service. The post holder will provide holistic and autonomous evidenced-based nursing care to patients, focusing on promoting patient safety, health, and wellbeing through timely assessment and intervention. They will work independently, delivering appropriate nursing interventions and evaluating outcomes, while collaborating with multidisciplinary teams. The role responds to urgent and unpredictable out-of-hours demand, prioritising patients and managing complex situations.

- Able to develop effective interpersonal skills/relationships across the health & social care setting.
- Good verbal & written skills.
Desirable criteria
- IT skills.
- Experience of using Electronic Patient Records.
- Audit and research.
- Supra pubic catheterisation.
- Tissue viability qualification.
- Leg Ulcer management qualification.
- IV Therapy
- Chronic disease management.
- Experience of
- working within palliative care.
- Management skills.
- Teaching & assessing skills.
- Previous community experience
- Post-registration nursing experience
- NMC registered adult nurse
- Evidence of level 5/6 study.
- V100/V150 nurse prescriber
- Teaching and Assessing qualification

Job summary
Are you a registered
nurse looking for a role in District Nursing. We are looking for an experienced
and motivated individual who will be responsible for carrying out holistic and
autonomous evidenced-based nursing care to patients under the care of the
Warrington Out of Hours District Nursing service. You should be able to
demonstrate ongoing professional development.
The post holder will
provide a high standard of care, utilising clinical skills in the assessment,
planning and implementation of pro-active care to a community population with
nursing needs referred into the district nursing service. The post holder will
work in close collaboration with other professionals/stakeholders within the Warrington
footprint.

Main duties of the job
The OOH role focuses on promoting patient safety, health

needs, delivers appropriate nursing interventions and evaluates outcomes. They
work autonomously and without direct supervision, demonstrating clinical
competence
