An exciting opportunity has arisen for an experienced RSW to join our Community Rehabilitation Team (CRT) in the St AndrewsLocality.

The CRT provide rehabilitation to the adult population within the locality. This will be delivered to service users as in-patients out-patients or within their own home.

The primary function of this role will be to work with the Occupational Therapy and Physiotherapy teams to deliver therapeutic treatment programmes to service users in their own homes, preventing hospital admission and supporting early discharge from hospital. Occupational Therapists and Physiotherapists will provide specialist assessments to this patient group and allocate work to this post holder. This post will primarily deal with the function of daily rehab in the community, a service provided 365 days of the year from 0745-2200.

You will have an SVQ Level III in Health and Social Care or equivalent. Experience working as a support worker would
