Role summary
This Keyworker Support Worker role involves supporting autistic children and young people, and those with a learning disability, who are at risk of hospital admission. You will work alongside a Keyworker to deliver personalised support through visits and interventions, building positive relationships with families. The role focuses on helping families navigate complex services and ensuring the child's voice is heard in their care. You will contribute to multi-agency working and maintain accurate records to support community-based care. Lived or learned experience of Autism and/or Learning Disabilities is essential.
Essential criteria
- Good general education (e.g., GCSE English & Maths or equivalent)
- Willingness to complete all mandatory training, including Breakaway and safeguarding
- Commitment to ongoing personal and professional development
- NVQ Level 3 in Health & Social Care, Childcare, Education, or related field, or equivalent
Desirable criteria
- Training in autism, learning disability, trauma-informed practice or de-escalation or other relevant training for this post
- Previous relevant experience working with Autistic people and/or those with Learning Disabilities
- Previous relevant experience with Children and Young people and their families
- Knowledge of risk assessment principles
- Experience in providing support for people in the community
- Knowledge and understanding of safeguarding procedures
- Knowledge and understanding of Mental Capacity and Best Interest
- Experience supporting individuals who may be distressed, dysregulated or present with challenging behaviour
- Experience maintaining accurate written or electronic records
- Experience in providing support to autistic children and young people, and/or those with a learning disability, who are risk of hospitalisation
- Experience of multi-agency working (health, education, social care, voluntary sector)
- Experience supporting CYP and families to express their views and be heard
- Experience in lone working
- Ability to communicate sensitively and effectively with CYP and families, including those in distress
- Ability to adapt communication to age, developmental needs, autism/LD profiles and communication differences
- Ability to work independently and as part of a team, knowing when to seek support
- Ability to support CYP and families to express their voice, views and preferences
- Ability to recognise escalating risk and seek guidance appropriately
- Ability to prioritise workload, manage competing demands and respond to changing situations
- Accurate record-keeping and confident use of IT systems (e.g., electronic records, MS Teams)
- Ability to build effective relationships across agencies
- Skills in de-escalation, trauma-informed or relational approaches
- Ability to contribute meaningful observations to assessments or care planning

Job summary
Following a recent service review, we are delighted to introduce Keyworker roles as part of our enhanced non-clinical offer. We will be recruiting to one position, aligned to locality areas:
Grantham - Sycamore Centre
You will support autistic children and young people, and those with a learning disability, who may be at risk of hospital admission. Working alongside a Keyworker, you will help deliver personalised support through visits, interventions and ongoing engagement with families.
You will build positive relationships, support families to navigate complex services, and ensure the voice of the child or young person is heard and reflected in their care. The role includes contributing to multi-agency working, maintaining accurate records, and supporting coordinated, community-based care.
This is a rewarding opportunity to make a meaningful difference, helping families feel supported, informed and confident. You must have either lived or learnt experience of Autism and/or Learning Disabilities.
Main duties of the job
Support the delivery of person-centred care plans for children, young people and families through direct work, including visits, interventions and ongoing support.
