Role summary
This role is for a Care Home Practice Foundation Pharmacist within the Care Home GP Practice, part of Ealing Community Partners. The post holder will work in a multi-disciplinary team, undertaking clinical medication reviews for patients, especially those with polypharmacy and multiple co-morbidities in care homes. Responsibilities include supporting the pharmacy team with prescription queries, managing repeat prescriptions, and providing advice on clinical medicines issues. The role also involves contributing to medicines optimisation and quality improvement initiatives.
Key responsibilities
- Undertake clinical medication reviews for patients, particularly those with polypharmacy and multiple co-morbidities in care homes.
- Provide support to the pharmacy team and care home general practice staff regarding prescription and medication queries.
- Help support the repeat prescriptions system and deal with acute prescription requests.
- Perform medicines reconciliation on transfer of care and implement systems for safer prescribing.
- Provide advice on clinical medicines issues, addressing both public and social care needs of patients.
- Provide clinical advice on medicines optimisation and quality improvement.
Essential criteria
- Mandatory registration with General Pharmaceutical Council (GPhC) with independent prescribing annotation
- Membership of the Royal Pharmaceutical Society/ PCPA
- Masters degree in pharmacy (MPharm) (or equivalent)
- Working towards specialist knowledge acquired through post- graduate diploma level or equivalent training/experience
- Therapeutic and clinical knowledge and understanding of the principles of evidence based healthcare.
- Is able to plan, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
Desirable criteria
- Working towards Post-registration Foundation Pharmacist Credentialing
- Enrolled, or working towards equivalence to Primary Care Pharmacy Education Pathway from Centre for Pharmacy Postgraduate Education

Job summary
Thank you for your interest in this post for as a Care Home Practice Foundation Pharmacist in the Care Home GP Practice.
The Care Home GP Practice is part of Ealing Community Partners (ECP) and provides an enhanced primary care service to Care Homes (Nursing and Residential) across Ealing. The team includes GPs, Pharmacists, Nurses, Advanced Practitioners and Administrators and the service is provided 8am - 8pm 7 days per week.
The service works closely with colleagues across ECP and Ealing's Borough Based Partnership teams as well as Care Home providers with the aim of providing an integrated approach to the care available to the residents.
The service has transferred to the Trust, from a previous subcontracted arrangement, and this will give the opportunity to explore the current and future delivery of the service via a transformation programme.
Main duties of the job
The ECP Care Home GP Practice provides enhanced primary care services for over 1000 patients in nursing homes and care homes in the borough of Ealing.
The post holder is a pharmacist, who acts within their professional boundaries, working as part of a multi-disciplinary team in a patient-facing role. The post holder will assist in areas of patient care and chronic disease management within the care
