Role summary
This role is for a Healthcare Assistant to join an expanding multi-disciplinary frailty team, delivering care in Proactive Frailty Care Planning Clinics across Buckinghamshire. The postholder will assist patients and clinicians in conducting Comprehensive Geriatric Assessments to identify and solve problems for moderately frail patients. The aim is to provide necessary solutions to help patients lead healthier, more independent lives. The role involves supporting patients within agreed protocols and clinical guidelines, working within competence as part of a multi-disciplinary team.
Essential criteria
- oExperience of working with people, ideally in a front-line customer service role.
- oExperience of providing person centred care.
- Skills, Abilities & Knowledge
- oAn understanding of the importance of patient confidentially and privacy.
- oDemonstrates a caring, compassionate and empathetic approach to patient's and colleagues, whilst having a professional approach.
- oRecognises how patients and colleagues need to be treated with dignity and respect and can demonstrate these behaviours in their daily work.
- oGood verbal and written English communication skills.
- oBasic knowledge of health and safety with an awareness of the importance of following Trust policies and procedures.
- oAn appreciation of equality, diversity and inclusion and how this influences how care is provided to patients.
- oAbility to work as part of a team
- oA willingness to provide the highest standards of patient care including personal care.
- oUnderstands the limit and scope of their job role and the need to only do tasks they are trained to do.
- oKnows when to escalate to a registered nurse when the situation is beyond their scope of practice.

- oLevel 2 or Level 3 (NVQ) in health and Social Care or equivalent relevant experience
- oWilling to work towards Level 2 functional skills (Math & English)

Job summary
The opportunity has arisen to be part of our expanding multi-disciplinary frailty team and deliver care in our Proactive Frailty Care Planning Clinics across the six Integrated Neighbour Team locations in Buckinghamshire.
In our Proactive Frailty Care Planning clinics, we conduct Comprehensive Geriatric Assessments (CGAs) on moderately frail patients to identify and solve problems before things go wrong.
The postholder will assist patients and clinicians within the multip-disciplinary team to enable assessments to take place effectively to provide the necessary solutions to ensure patients can lead healthier, more independent lives.
Main duties of the job
The Healthcare Assistant will support patients within agreed protocols and clinical guidelines, working within their own competence as part of a multi-disciplinary team assisting clinicians to undertake frailty health assessments.
- To assist the registered clinicians in the delivery of high quality and person-cantered care.
- To provide fundamental nursing care, e.g., assistance with personal hygiene, eating and drinking, mobility needs, washing under direct or indirect supervision.
