Role summary
This role is for an enthusiastic, compassionate, and highly organised Band 6 Renal Transplant Co-ordinator to join the Renal Team at NNUH. You will support patients throughout their entire transplant journey, from assessment and preparation to long-term follow-up. The role involves coordinating complex pathways, providing specialist advice and psychological support, and acting as a key advocate for patients and their families. You will also have the opportunity to develop and expand the Living Donor Programme.
Key responsibilities
- Manage a specialist caseload across the transplant pathway, coordinating assessments, investigations, follow-up care and education.
- Provide highly specialist clinical advice and lead nurse-led transplant and donor clinics.
- Prioritise and manage complex transplant patient caseloads, identifying when urgent review, intervention or escalation is required.
- Plan and deliver nurse-led post-transplant follow-up care, including clinical assessment, patient education, referrals and ongoing monitoring.
- Assess and determine the suitability of potential living kidney donors and coordinate appropriate investigations.
Essential criteria
- RN Diploma HE or Degree pathway
- Extensive post registration experience including significant experience in relevant clinical area
- Relevant specialist clinical/academic knowledge
- Significant experience at band 5 in a Renal speciality
- Management / Leadership experience
- Leadership skills
- Effective communication skills
- Attitude, aptitude
