Role summary

This is a 12-month fixed-term Band 3 Clinical Support Worker role within the CYPF Intensive Support community team. The role involves providing high-standard, person-centred care to young people with learning disabilities and/or autism, under the supervision of nursing staff and allied health professionals. Job summary

An opportunity has arisen to offer a 12 month Fixed term Band 3 role within our CYPF Intensive Support community team supporting young persons with a learning disability and or Autism and their families who come in to contact services at difficult times in their lives

Main duties of the job

To provide a high standard of person-centred care under the supervision of nursing staff and allied health professionals, to young people with learning disability diagnosis who may have complex needs and/or behaviours that challenge.

To support carers and families in maintaining a safe care environment in line with Positive behaviour support approach. To reduce the risk of placement break down and admission to hospital setting.
