Job summary

An exciting opportunity has arisen for a skilled and motivated Physiotherapist to join the Integrated Reablement Team, incorporating the Urgent Community Response (UCR) Service, Pathway 1 Discharge to Assess (D2A) and Reablement Services across Nottingham City.

Working within a multidisciplinary team of Advanced Clinical Practitioners, Physiotherapists, Occupational Therapists, Nurses, Pharmacy Technicians, Assistant Practitioners, Rehabilitation Support Workers and Admin Staff, you will support patients to maximise independence through rehabilitation and crisis intervention within their own homes. The service aims to prevent unnecessary hospital admissions and facilitate timely, safe hospital discharges.

As part of the UCR service, you will respond to urgent referrals from emergency and out-of-hours services, providing a rapid two-hour response to crises such as falls, functional decline and acute illness. Comprehensive in-house training will be provided, including falls recovery, safe assistance from the floor and enhanced clinical assessment skills.
