Role summary
This role is for a Band 6 Frailty Nurse or Paramedic to join the Unplanned Care team within Hertfordshire Adult Community Services. - Experience of assessing and providing quality care to people with long term conditions

- Experience of working in a multidisciplinary team
- Experience of supervising staff and completing appraisals
- Skills & Knowledge
- Ability to communicate effectively verbally and in writing
- Ability to make decisions and prioritise actions
- Ability to manage own workload and plan the workload of others, to work autonomously
- Basic IT skills including use of Microsoft Office package and email
- Clinical knowledge across a range of long term and enduring conditions
- Knowledge of evidence-based practice and able to implement in practice
- Knowledge of clinical governance processes, assessing and managing risk
- Knowledge of current NHS organisational structures and policy initiatives

CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!
We are looking for a Band 6 Frailty Nurse or Paramedic to join our supportive and dedicated team within unplanned care.
Unplanned Care provides multidisciplinary services and includes Discharge to Assess Therapy Teams, Early Intervention Vehicle, Frailty Hospital at Home, Rapid Response and Overnight Nursing. The post will be aligned with one service, primarily, but staff within Unplanned Care work flexibly across services as required.
You will deliver high-quality, short-term care programmes for adults referred to the Frailty Hospital at Home service across West Hertfordshire. You will provide care in patients' own homes, supporting those with acute health needs, helping to prevent unnecessary hospital admissions, and enabling timely discharge from urgent care settings.
