Role summary
This role is for a Senior Social Prescriber Link Worker to cover maternity leave for 12 months. You will work with GP practices and multidisciplinary teams to support patients aged 18+ with non-medical needs like long-term conditions, loneliness, mental wellbeing, debt, and housing concerns. Essential criteria

We have an exciting opportunity as a Band 5 Senior Social Prescribing Link Worker to cover
Maternity Leave for 12 months.
In this role, you'll work
closely with GP practices within PCNs and the wider multidisciplinary team to
support people whose health and wellbeing are affected by social, emotional or
practical issues.
You will help patients aged 18+ navigate non-medical needs
such as long term conditions, loneliness, mental
wellbeing, debt, housing concerns, or physical inactivity. Rather than focusing
purely on medical symptoms, you will give people the time and a safe space to
explore "what matters to the patient" and connect them to empowering
local community groups and support networks. You will also have line management
responsibilities within this role.
