Role summary
Join our Community Learning Disability Team in Plymouth as a dedicated Community Nurse. This role involves assessing, planning, coordinating, and reviewing the care of adults with learning disabilities, addressing both physical and mental health needs. You will work holistically, managing cases, coordinating support, and liaising with various agencies to ensure person-centred care and reasonable adjustments. The position requires providing clinical support and contributing to high-quality, outcome-focused services across Plymouth.

- Ability to frequently travel independently and in a timely manner between Livewell Southwest sites and community locations including areas not served by public transport.
- Able to problem solve and be creative in care planning.
- Experience undertaking comprehensive health needs assessments.
- Working in multidisciplinary teams across health and social care.
- Experience of working with people with learning disabilities / mental health issues.
- Experience working in primary /community health settings.
- Experience in mentoring students.
- Working across organisational boundaries.
- Knowledge and understanding of the political dimensions of health and social care.
- Knowledge of Personalised Care Framework
- Good knowledge and understanding of the Mental Health Act, Mental Capacity Act, Deprivation of Liberty and reasonable adjustments.
- Understanding of the Safeguarding adults and children processes and relevant issues
- Able to demonstrate understanding and competency in assessment, care planning and review.
- Knowledge of recent developments in Learning disability/Mental Health care.
- Understanding of the complexities of physical health care needs in adults with Learning Disability.
- Understanding of the work and role of the independent sector and care homes in provision of long-term care.
- Understanding of the continuing care eligibility for fully funded NHS care.
- Demonstrate an understanding of the Primary Care service delivery.
