Role summary
This role is for a District Nurse Deputy Team Leader in Powys, working autonomously in a rural setting to provide diverse nursing care. The post holder will deliver skilled, autonomous nursing care to patients in their homes, including palliative care, end-of-life support, chronic condition management, and wound care. It's an opportunity for a registered nurse to broaden skills within a multi-disciplinary team, with support for Welsh language development.
Key responsibilities
- Participate in the delivery of skilled, autonomous nursing care to patients within their own home.
- Provide palliative care support and assessment.
- Deliver care at the end of life.
- Support and assess individuals and carers with long-term health conditions.
- Promote self-management skills.
- Provide wound care.
Essential criteria
- NMC Registered Nurse
- Post registration District Nursing qualification (BSc in relevant subject)
- Expeience of proactive caseload management of wide range of patients including those with chronic conditions
- For Band 6 Annex 21: to already be undertaking the 2 year qualification for DN SPQ
- Significant clinical experience in assessing, planning, implementing and evaluating cae for the patient group of the caseloadExperience in a wide range of clinical skills
- A high level of interpersonal and communication skills
- Standard keyboard skills and IT literacy
- Ability to engage with members of the public
- Ability to work independently and as part of a team
- Ability to diffuse difficult/threatening situations
- Ability to deal with complex/stressful situations
- Able to work in a multi/disciplinary/multi-agency arena
- Excellent organisational skills
- Flexible working pattern
- Post is community base, the ability to travel throughout the locality in a timely manner is essential
Desirable criteria
- Independent prescribing qualification
- MSc Advanced clinical Practice
- Management/leadership qualification or equivalent preparation
- Experience at management level demonstrating ability to deliver improvements to service users care
- Competence and confidence in a wide range of clinical skills including palliative care, chronic disease management and wound management
- Ability to speak or learn Welsh to a satisfactory level
- Local knowledge of statutory and voluntary services
