Role summary
This role is for a Specialist Physiotherapist within the @home service, a leading Urgent Community Responses service in London. You will provide specialist assessment, intervention, and rehabilitation to support patients in their homes, aiming to avoid hospital admissions and facilitate early discharge. The role involves working in a multidisciplinary team across Lambeth and Southwark, with a shift pattern including weekends. You will also have supervisory and teaching responsibilities for junior staff and students.
Essential criteria
- Degree in Physiotherapy
- Evidence of CPD including attendance at recent post graduate courses relevant to the clinical field
- Health and Care Professions Council (HCPC) registration
Desirable criteria
- Member of CSP
- Member of Special Interest Group
- Broad postgraduate experience across a range of clinical areas including working with adults and older people with chronic conditions and complex co-morbidities
- Rehabilitation experience minimum Band 5 in all core areas: neurology, orthopaedics, respiratory, and care of the elderly
- Experience of working as member of a multidisciplinary team
- Previous experience in rapid response, acute or urgent care setting
- Rotational experience including community working
- Supervision of Band 5, Physiotherapy Assistant or Students
- Experience of teaching in-service programmes
- Demonstrated ability to use evidence based practice (including clinical outcome measures) to evaluate patient outcomes
- Demonstrated ability to work effectively under pressure with frequent interruptions
- Able to travel across boroughs via public transport
- Evidence of excellent time management, prioritisation and organisational skills
- Evidence of participation in research / audit

Job summary
The @home service is one of the leading and largest Urgent Community Responses services in London and we are looking for a Specialist Physiotherapist to join the team. This role is suited to an experienced Band 5 Physiotherapist ready for growth into a Band 6 role; or current Band 6 Physiotherapist seeking a new and rewarding challenge. This person will be innovative, enthusiastic, and thrive working in a multidisciplinary service.
The service aligns with the NHS England's Long-Term Plan to support England's ageing population and those with complex needs. The @home service are expanding our pathways and successful candidates will have a passion for working under an admission avoidance model and in facilitating early discharge from hospitals.
The service operates seven days per week between the hours of 08.00hrs to 23.00hrs however therapists will work until 20.00. This is a shift work pattern rota of 13 shifts over a four-week period, inclusive of weekends.
This role requires the use of public transport for home visits. Alternatively having a valid and current UK driver's license and/or vehicle that can be used for home visiting is desirable.
