Role summary
This role involves supporting individuals with mental illness to develop, retain, or regain independent living skills. You will work collaboratively with Care Coordinators in an Assertive Outreach Team or support individuals in acute mental health crisis within a Crisis Resolution and Home Treatment Team. The position requires working closely with patients, families, and carers as part of a multi-disciplinary team, offering compassionate care and contributing to a positive experience for service users. Essential criteria
- Level 2 qualification or equivalent in a relevant area gained through experience, on the job training, short courses and completion of work based competencies

Job summary
Hampshire and Isle of Wight Healthcare NHS Foundation Trust have exciting opportunities that will enable you to gain more experience of supporting people to develop, retain or regain independent living skills that have been disrupted through the experience of living with mental illness.
Assertive Outreach Team to work collaboratively with Care Coordinators to co-produce good quality assessments and interventions to service users who are experiencing severe mental illness and find it difficult to engage with mainstream services. This is 22.5 hours a week.
Crisis resolution and home treatment team supports individuals who are experiencing acute mental health crisis and who require intensive input in order to remain in the community, averting hospital admission where possible. Whilst also supporting individuals along their journey through the Acute Care Pathway from the hospital environment to home. Service hours are Mon-Fri 9-5 or 2-10 with alternate weekends.
