Role summary
This role is for a registered Community Nurse to join an Integrated Care Team, providing assessment, implementation, and evaluation of care for patients in their own homes. The post holder will manage a defined caseload of housebound patients within planned pathways and respond to clinical priorities in unplanned pathways. They will act as a professional role model, upholding organisational values and facilitating a culture of trust and support. The nurse will also be proactive in problem-solving, including ordering equipment and liaising with multidisciplinary teams.
