Essential criteria

  • oRegistered General or Mental Health Nurse with significant post qualification experience.
  • oA wide range of significant clinical experience including hospital and community settings at a senior staff nurse level.

  • oAbility to plan, organise, review wide range complex activities and to provide and operational lead in their internal implementation.
  • oExperience of multi-agency and multi-disciplinary working.
  • oAbility to travel independently to include working between several different locations on a rotational basis including potential on-call duties.
  • oAble to demonstrate an understanding and knowledge of community services.

Job summary

A rare and exciting opportunity has arisen for an enthusiastic and self-motivated individual to join the South Staffordshire Hospital Discharge Team based at Great Wyrley Health Centre.

The role of the Discharge Liaison Sister / Charge Nurse will be based in our Triage hub, providing clinical expertise to our referring acute hospital trusts. The main referring hospitals in the South of the county are Russell's Hall Hospital, Newcross Hospital, Walsall Manor Hospital, Good Hope Hospital and Queens Hospital Burton.

Main duties of the job

The successful candidate will be part of a triage team that receive referrals from inpatient wards for patients with complex needs who require ongoing support following hospital discharge. The role includes the triaging of patients' needs as part of a multi-disciplinary team with a key role in decision making. Part of the role of the Discharge Liaison Sister / Charge Nurse will be to support end of life patients to receive the best quality care in the place of choice determined by the patient.
