Role summary

This role is for a highly motivated Staff Nurse to join the Discharge Lounge team at Huddersfield Royal Infirmary. The successful candidate will be responsible for transferring and accepting patients, coordinating discharge planning, and liaising with care providers and families. Job summary

We are looking for a highly motivated, adaptable individual to join our team on the Discharge Lounge, Integrated Hub at Huddersfield Royal Infirmary

The successful candidate will need to be able to use their own initiative and work well within a team.

The discharge lounge is a busy, pro active ever changing environment, diverse and challenging .

Main duties of the job

Transferring and accepting patients from every ward area, to the discharge lounge into the integrated hub.

Coordinating discharge planning of the client, liaising with care providers, care facilities and family members. Recognising the need for further community referrals to safeguard patient safety and create an effective discharge .

Completing patient education with regards to care providers, the discharge plan, medication management and any follow up arrangements.

Effective communication and excellent documentation on the electronic patient record

Whilst in our care, completing hygiene, nutrition, hydration, elimination and mobility needs. Engaging with the patients throughout their time in the hub, offering comfort, compassion and reassurance.
