Role summary
This role is for a registered nurse passionate about improving outcomes for people with severe mental illness. You will join the expanding Physical Health and Wellbeing Service, focusing on specialist clozapine care and physical health monitoring. The service aims to reduce health inequalities and deliver care closer to home for individuals with severe mental illness. It's a unique opportunity to develop specialist skills in clozapine care, medicines optimisation, and long-term condition management within mental health services.
Essential criteria
- Nurse & Midwifery Council (NMC) active registration appropriate to the role
Desirable criteria
- V100 (prescribing for nurses) or equivalent
- Relevant qualifications in e.g. leg ulcer management, care of the dying, diabetes.
- Teaching/Assessing in clinical practice.
- Experience of managing patients with long-term conditions
- Show an awareness of current health and social care policies
- Previous experience of community health care.
- Ongoing professional development and knowledge suitable to this service area
- Knowledge of specialised nursing practices specific to this service area, e.g. wound management/ compression bandaging competencies, acute mental illness and treatment needs.

We have an exciting opportunity to join our expanding Physical Health and Wellbeing Service, supporting the delivery of specialist clozapine care and physical health monitoring across Bradford District Care NHS Foundation Trust.
This innovative service sits at the interface of mental and physical healthcare and plays a vital role in improving access to clozapine, supporting community-based treatment, reducing health inequalities and delivering care closer to home. The service supports people with severe mental illness through physical health assessment, specialist medicines monitoring, health promotion and integrated multidisciplinary care.
We welcome applications from nurses registered in either Mental Health Nursing or Adult Nursing and would particularly welcome applications from individuals who are registered in two fields of nursing practice with the NMC.
This is a unique opportunity to develop specialist skills in clozapine care, medicines optimisation, physical health and long-term condition management within mental health services.
