Role summary

Essential criteria
- Subject to reasonable adjustments in respect of any disability, the post holder must possess a valid driving license and be able to drive and have the unrestricted use of a car or motorcycle for work at all times including community based work.
- Minimum 2 years experience working in a role delivering care in a mental health setting
- Foundation Degree in Mental Health or related subjectOR Level 3 Qualification Health related area and 2 years' experience in a care role OR 5 years' experience in a role delivering care in a mental health setting
- Driving license suitable for use in the United Kingdom, access to a vehicle will also be required.

Job summary

To work as part of a multi-disciplinary team, delivering health and social care, focusing on the crisis intervention of people experiencing mental health problems. Working autonomously with an allocated caseload, under supervision of a qualified clinician.
We are looking to appoint a team-player who is motivated and enthusiastic, committed to the delivery of the highest quality and excellence in mental health care.
Promoting good physical health is a key area in mental health, and our Assistant Practitioners play in a vital role in supporting the team, including the facilitation of physical health assessments and healthy lifestyles work in the community setting.
Main duties of the job
Assess, plan, organise and deliver care as agreed with supervising clinician, in line with clinical governance
Undertake and record Physical Health checks particularly during clozapine titration and support the physical well-being of people under our care
Lead on the support for carers in the team ensuring that all carers are identified and supported whilst under the care of the crisis and home treatment
