Role summary
This role is for a Band 5 Registered Mental Health Nurse to work as a Community Practitioner in a community setting. You will deliver evidence-based care for service users with mental health conditions in their homes and various accommodation settings. The post involves comprehensive assessment, planning, implementation, and evaluation of person-centred care, working collaboratively with a Multi-Disciplinary Team and supervising junior staff. The service focuses on medium to longer-term mental health care for Lambeth residents aged 18 and over.
Essential criteria
- BSc (Hons) in Nursing or Diploma in Mental Health Nursing
- Preceptorship course (or other relevant teaching/assessment course).
- Ability to assess formulate and plan care
- An ability to develop therapeutic relationships with service users, identifying their care needs
- Experience of working in community mental health settings (i.e. Home Treatment Team, CMHT) or acute inpatient settings
- Experience in managing safeguarding issues
Desirable criteria
- Experience of working in an MDT setting
- Experience of managing a caseload.
- Knowledge / Skills/Abilities
- A knowledge and understanding of the needs of people with mental health problems
- Current up to date knowledge of relevant legislation.
- Awareness of bio-psycho-social assessments and the safe and effective management of risk
- Skilled in the communication of sensitive issues
- Skilled in the assessment and monitoring of common physical health problems and able to deliver appropriate care
- Knowledge of wider local organisations to support recovery plans for service users Ability to role model, reflect and provide evidence of conflict resolution
- Personal attributes
- Recognition of own limitations, strengths and weaknesses and an ability to seek advice when necessary
- Ability to use computerised information and communication systems

Job summary
This post is open toBand 5 Registered Mental Health Nurses who wish to broaden their experience and further develop their assessment and care planning skills in a community setting. There will be an expectation for the post holder to have completed, be in the process of completing, or be willing to attend the Trust Preceptorship.
Focussed Support service offer medium to longer term mental health care to residents in the Borough of Lambeth aged 18 and over regardless of diagnosis using the Care Programme Approach. We are a part of the Lambeth Alliance, and we have a close interface with a variety of SLaM services, other statutory organisations (inc. social care) and voluntary sector.
The Lambeth Living Well Network (LWN) Alliance brings together Certitude, Lambeth Clinical Commissioning Group, Lambeth Council, South London & Maudsley NHS Foundation Trust and Thames Reach organisations to deliver an innovative way of providing mental health support for those who need it in the Lambeth Borough; a fully integrated and coordinated mental health system which focuses on people's strengths and supports their whole health and wellbeing.
