Role summary

This is an exciting opportunity for a Band 3 Complex Discharge Tracker within the Home First (Discharge to Assess) team in East Staffordshire. The role involves scheduling and allocating care calls on the Complex Discharge Tracker System, working with Home First Locality Teams to ensure timely allocation of referrals within discharge to assess timescales. You will be responsible for validating and updating the system for each locality. The position requires an enthusiastic, passionate, and dynamic individual who is motivated, flexible, and an effective team player, with occasional face-to-face care/support for community patients.

Essential criteria

  • GCSEs or equivalent Certificates/Qualifications in Administration
  • Ability to effectively maintain, update and sustain systems
  • Excellent communication skills
  • Demonstrate a commitment to continuous professional development

Desirable criteria

  • NVQ Level 3 in Care or equivalent qualification or experience.
  • Significant experience of working in health / social care environment
  • Experience of working within the constraints of legislation, regulation, policy and procedures
  • Experience of delivering improvement to consistently achieve and exceed personal and team objectives
  • Experience and ability to work collaboratively
  • Experience of work planning & allocation, effective coordinator of allocated care calls to meet the service delivery

Job summary

We are offering an exciting opportunity for a Band 3 Complex Discharge Tracker for the Home First (Discharge to Assess) in East StaffordshireThe Team is an integrated clinical and care and support service providing short term reablement intervention to enable people to maximise their independence and meet their health and social care needs.

You will be responsible for scheduling and allocating care calls on the Complex Discharge Tracker System and will be working in partnership with the Home First Locality Teams to ensure that all referrals are allocated care calls within the discharge to assess timescales andwill ensure that the Complex Discharge Tracker system is validated and updated within each locality teams.

We are looking for a team member who is enthusiastic, passionate and a dynamic individual. You will need to be motivated, flexible and an effective team player, in order to prioritise and implement effective negotiation and communication skills. There will be a requirement to deliver face to face care/support to our community patients on an adhoc basis.
