Role summary
This permanent Community Nurse role in the Forest of Dean involves providing high standards of care and compassion to a diverse patient population in their own homes. You will be responsible for implementing and undertaking patient-centred care, managing your own workload, and contributing to individualised development planning. The role requires excellent communication skills and the ability to make clinical decisions, working across a 7-day shift system.
Essential criteria
- Registered Nurse with NMC
- Degree or Diploma in Nursing or equivalent
- Willingness to undertake additional training as required.
- Full driving licence and use of vehicle
- LENGTH AND / OR NATURE OF EXPERIENCE
- Experience of working without direct supervision
- Experience of working with student learners in practice
Desirable criteria
- Previous experience of community nursing practice
- Evidence of post-registration education & training.
- Holistic health assessment course (or equivalent)
- Teaching & assessing qualification
- PROFESSIONAL / MANAGERIAL / SPECIALIST KNOWLEDGE
- Ability to work on own initiative, assess patients competently and report back to Team Leader (or appropriate health care professional) relevant changes in patient's condition.
- Ability to effectively manage the workload, prioritise and delegate work in the absence of the Team Leader - District Nurse / Senior Community Nurse
- Ability to liaise effectively within the Primary Health Care Team and with other professionals / agencies
- Demonstrable skills in risk assessment and management

We are currently seeking nurses possessing the highest standards of care and compassion in order to aid, coach and support our diversepopulation of patient requirements here in the Forest of Dean.
As a Registered Community nurse, you will be placed in the privileged position of caring for patients in their own individualised environment and responsible for the implementation and undertaking of patient centred care.
Our collaboration with our Integrated Community Team colleagues is something we pride ourselves on within the Forest of Dean locality. Inaddition, Gloucestershire Health & Care Foundation Trust Community Nurses are placed in possession of: -
