Role summary

This role is for an enthusiastic and highly organised Clinical Pathway Navigator to support the CAS triage pathway across Hepatology, HPB, General Surgery and Colorectal Surgery within the Liver and Digestive Health directorate. The postholder will guide patients through their hospital care journey, acting as a single point of contact to coordinate their care from GP referral to discharge. This involves ensuring all administrative functions align with specific care pathway instructions and assisting patients with multiple specialist needs to coordinate appointments across the Trust.

Essential criteria

  • o Demonstrable ability to meet the Trust shared commitments
  • English and maths at GCSE level or equivalent
  • Knowledge of medical terminology

  • Diploma level qualification or equivalent experience
  • Courses/further study attended to demonstrate evidence of personal development
  • Previous experience of working with the public and/or patients in a healthcare environment
  • Experience of interaction with all levels of staff
  • Experience administrative and organisational experience
  • Experience of managing and prioritising own workload and working to deadlines

Main duties of the job

The postholder will support patients through their hospital care journey, providing the patient with a single named point of contact who will coordinate their care. Work as part of a team within a dedicated speciality/patient group to ensure that all administrative functions for patients from the time of GP referral to the time of discharge are coordinated in line with specific care pathway instructions to facilitate the smooth running of the patients' journey. Where a patient has multiple specialist needs, assist the patient in coordinating their pathways to reduce multiple appointments across the Trust.
