Role summary

This role is for a District Nurse with a Specialist Practice Qualification (SPQ) to lead and manage a team within the Cheltenham Integrated Community Team. You will be responsible for direct patient care, professional leadership, and operational management of the Community Nursing Service, managing a defined caseload and supporting a team of nurses. The position requires extensive community nursing experience, excellent communication skills, and critical thinking for triage processes, working collaboratively within a multidisciplinary team.

Key responsibilities

  • Responsible for direct patient care delivery, professional leadership and operational management of the Community Nursing Service.
  • Direct management and leadership of a caseload of patients attached to affiliated GP practices.
  • Direct management and leadership of a team of nurses and unregistered colleagues in the delivery of outstanding community nursing care.
  • Acting as a pivotal part of the Multidisciplinary Team within the Integrated Community Teams.
  • Illicit an accurate, holistic assessment of patient needs as part of a case management approach.
  • Work within a dedicated referral centre and possess critical thinking & negotiating skills to aid the triage process.

Essential criteria

  • Evidence of SPQ
  • Registered Nurse (Adult/RGN)
