Role summary

This is an exciting opportunity for a Band 4 Assistant Practitioner to join a new Neighbourhood Rehabilitation Pathway within East Cheshire NHS Trust. The role involves working as part of an integrated community team to support early hospital discharge and help patients regain independence at home. The successful candidate will deliver therapy independently or jointly with therapists in various settings, managing a delegated caseload under supervision. Flexibility to work across multiple sites in East Cheshire is required.

Key responsibilities

  • Work under the supervision of a qualified staff member, as well as autonomously within trust policies and scope of practice.
  • Maintain accurate and comprehensive patient records.
  • Provide a comprehensive rehabilitation service for patients across East Cheshire.
  • Deliver therapy independently or jointly with therapists in individual, group, and multidisciplinary settings.
  • Support aspects of rehabilitation care provision with partner care agencies.
  • Efficiently manage delegated caseloads.

Essential criteria

  • 5 GCSE's including English and Maths or equivalent
  • BTEC National Diploma or equivalent in a relevant subject
  • Knowledge / competency of a range of assessment tools & interventions relevant to the role
  • An understanding of confidentiality & Information Governance
  • Effective communication and listening skills
  • Takes own initiative and seeks appropriate guidance
  • Ability to work as part of a team
  • Good organisational skills
  • Ability to teach practical skills
  • Experience of contributing to individual & group interventions for patients & settings relevant to the role
  • Skills in delivering therapy programmes
  • Ability to adapt programmes to meet the changing needs of the clients with reference to the supervising therapist
  • Ability to support and motivate patients as appropriate
  • Ability to carry out and complete delegated project
  • Ability to meet travel requirements

  • Evidence of working as a therapy assistant

East Cheshire NHS Trust is pleased to offer an exciting opportunity for a suitably experienced Band 4 Assistant Practitioners to join our newly established Neighborhood Rehabilitation Pathway. We welcome applications from individuals with a strong interest in working as part of an integrated community team.

There are two posts available across East Cheshire, with bases at Macclesfield, Congleton, Knutsford, Handforth and Poynton. The successful applicants will be expected to work flexibly and support colleagues across different sites within East Cheshire. As a result, the ability to travel efficiently between sites, including rural locations, is essential.

Applicants currently working at Band 3 with relevant experience are also encouraged to apply.
