Role summary
This Staff Nurse role is for the Elderly Virtual Ward, focusing on providing complex care and admission avoidance in the community for frail older people. The successful candidate will deliver direct and indirect patient care, participate in assessment and evaluation of care programmes, and contribute to a professional nursing culture. The role requires a highly motivated and adaptable individual with excellent communication skills, working in a fast-paced environment. Flexible working is expected as it is a 24-hour service.
Essential criteria
- Work as a Band 5 nurse in the community setting
- Clinical experience in the speciality of the post (frailty)
- Experience of completing Comprehensive Geriatric Assessment, IVs, catheter, wound care, rehabilitation, enablement
- Experience of working as part of a team with the ability to prioritise own workload
- Experience of working with patients, and their network from a range of diverse backgrounds and cultures
- IT literacy and ability to use NHS systems including SystmOne and EPR
- Excellent interpersonal and communication skills (face to face and written) with the ability to adapt to meet the requirements of patients, their support network and professional colleagues.
Desirable criteria
- Skills in research and audit methodology and application of findings to practice
- Understanding of Information Governance and Confidentiality
- Full UK Driving License
- Practise Assessor/Supervisor qualification or willingness to complete
- Registered Adult nurse
- Evidence of CPD
- Additional post-registration qualification relevant to field of practice
- Work as a Band 5 nurse in the community setting
