Role summary
This is a full-time Registered Mental Health Nurse / Care Coordinator position within a small, busy team. The role involves multidisciplinary working, fast-paced tasks, and close collaboration with service users and their families. Essential criteria
- oDip/BSc (Hons) in Mental Health Nursing
Desirable criteria
- Phlebotomy
- oCurrently Registered with appropriate professional body
- Evidence of further relevant post qualification training
- Practice Assessor training
- oPost registration experience as a Mental Health nurse preferably in a mental health setting working with older adults with functional and organic mental health needs.
- Experience of working with Older Adults
- Experience of carrying out assessments independently including risk assessment and mental capacity assessment
- Experience of Care Coordination
- Experience of carrying out individual work and group work
- oAbility to work independently and autonomously.
- Effective Medication Management skills
- Developing care plans with individuals and carers
- Experience of using Dialog + and co-producing care plans
- oAbility to undertake comprehensive risk assessment and implement risk management plans.
- oAbility to network, collaborate and liaise with other agencies.
- oEffective Physical health monitoring skills and knowledge of when to escalate concerns
- oHighly committed to teamwork
- oAbility to appreciate and apply continuous improvement strategies in practice.
- Motivated to learn to enhance knowledge and continuously develop skills.
