Role summary

This Care Co-ordinator role is a pivotal administrative, non-clinical position within primary care, acting as the interface between service users, carers, and various healthcare and social care organisations. The successful candidate will be part of North PCN, contributing to tackling health and social care inequalities, particularly for individuals with long-term conditions. The role requires excellent communication skills and a motivated approach to improve people's lives, with an emphasis on promoting independence and partnership-working.

Essential criteria

  • NVQ Level 3 Business Administration or equivalent
  • Diploma/HNC level in Business administration (or relevant experience)

Desirable criteria

  • Welfare rights basic training
  • Experience and Knowledge Required
  • Relevant experience of working with healthcare professionals/previous experience in the NHS or social care
  • Able to demonstrate a clear understanding of working with confidential information and an understanding of service user confidentiality
  • Working in a multi-disciplinary setting where influence and negotiation is required
  • Experience in use of databases
  • Vulnerable adults awareness
  • Experience of care of the elderly
  • Understanding of health and social care processes
  • Values, Drivers and Motivators
  • Willingness to undergo further training and development as the job develops
  • Requires a flexible approach, and to be highly motivated. The role may need to be reviewed and developed in the future in line with changing priorities

The Care Co-ordinator will be part of North PCN and provide an administrative, non clinical role, and applications are welcome from anyone with Health or social care experience.
