Role summary
This is a full-time, permanent Band 6 Mental Health Practitioner position within the Ealing Community Rehabilitation Service. The role involves providing integrated, personalised care for adults with complex psychosis in the community. Practitioners will manage a defined caseload, conduct clinical assessments, and coordinate mental and physical healthcare, focusing on relapse prevention and recovery. The role requires liaison with Local Authority and ICB colleagues to ensure suitable accommodation and support.
Key responsibilities
- Conduct clinical assessment and evidence-based interventions.
- Coordinate mental and physical healthcare, including relapse prevention and recovery-focused support.
- Provide psycho-social interventions to help service users manage problems and move towards individualised recovery.
- Carry continuing responsibility for a defined caseload of service users experiencing complex psychosis.
- Liaise with Local Authority and ICB colleagues regarding out-of-area and supported accommodation placements.
Essential criteria
- Professional qualification in Social Work, with current Social Work England registration or professionally qualified and registered Nurse with current NMC registration in Mental Health / LD/ Adult Nursing.
- Attained at least Level 2 Maths and English (equivalent to GCSE grade 9-4 (A*-C))
- Educated to degree or Diploma level.
- Demonstrable experience of post qualifying work as a Social Worker/Mental Health Nurse in a mental health setting
- Experience of working with patients and their carers with complex health and social care difficulties.
- Understanding and experience of operating with a comprehensive assessment framework
- Knowledge of counselling skills/cognitive behavioural skills, psychosocial interventions
- Knowledge of recovery approaches in mental health
- Advanced knowledge of care planning processes
- Specialist knowledge of mental health conditions as well as their relationship to physical health
- Experience of social work assessment and interventions
Desirable criteria
- Evidence of recent and on-going relevant further professional development.
- Experience of working with people within a social care model of mental health
- Evidence of experience in a Band 6 position or equivalent
- Experience of conducting a range of clinical assessments including risk assessments.

Job summary
Thank you for your interest in this Band 6 Mental Health Practitioner position within our Ealing Community Rehabilitation Service (CRS). This is a full-time and permanent position and attracts 15% Outer London weighting. We welcome applications from Registered Mental Health/LD Nurses (registered with the NMC) and qualified Social Workers (registered with Social Work England) who are interested in working in complex psychosis in the community.
Mental health practitioners play a key role in dissolving the barriers that can exist between mental health and physical health, social care, voluntary and community social enterprise organisations and local communities, and primary and secondary care, to deliver integrated, personalised, place-based and well-coordinated care for adults - moving towards a 'whole system' of mental health care, which will provide timely access to support, care and treatment.
We need qualified, experienced, skilled and professionally registered practitioners, to support people with mental health needs to access mental health care when and where they need it, and to be able to move through the system easily.
