Role summary

This is a Band 5 Physiotherapist role within the Routine Therapy/Community Recovery Service in South Warwickshire. The successful candidate will work with patients in their own homes, delivering integrated rehabilitation across community services. The role involves assessing and treating patients with complex physical, sensory, perceptual, and cognitive problems, working closely with a multi-disciplinary team. Flexible working is required as the service operates seven days a week.

Key responsibilities

  • To be professionally and legally accountable for a designated caseload of clients with a variety of physical and social needs.
  • To undertake a comprehensive assessment of clients with a wide variety of complex conditions using investigative and analytical skills.
  • To formulate individualised therapy programmes, using sound clinical reasoning skills and evidence-based treatment.
  • To have clinical responsibility for a designated caseload and at times dealing with unplanned and unpredictable urgent referrals.
  • To provide specialist advice and assessment for equipment requirements where more complex needs are identified.
  • To fulfil the duties of the key worker role, including detailed discussion with clients, families, or carers and developing comprehensive discharge plans.

Essential criteria

  • Degree or equivalent in physiotherapy.
  • State Registration with Health Care Professions Council
  • Clinical knowledge/ training through degree course /work experiences to support Band 5
  • Previous Band 5 experience
  • Experience of working within a multi-disciplinary team
  • Specialist knowledge and application of PT assessments and interventions
  • Comprehensive clinical experience knowledge of PT outcome measures
  • Good interpersonal and organisational skills and an ability to prioritise own workload
  • Ability to problem solve and manage an independent caseload of complex cases
  • Ability to comprehend and work within the Trust's policies and procedures, including data protection, equal opportunities and health and safety
  • Knowledge of relevant legislation and national priorities and current professional issues
  • Good written and verbal communication skills

South Warwickshire Foundation Trust is committed to bringing care closer to home a position has arisen for an enthusiastic Band 5 physio within our Routine therapy/Community recovery Service - South Warwickshire.

This is a fantastic opportunity to join a dynamic and forward thinking service. You will be well supported and encouraged to continuously develop to meet the needs of our service and your profession.

The successful candidate will work with patients in their own homes to deliver an integrated rehabilitation pathway across community services. You will liaise and work closely with Community & Specialist Nursing Colleagues, General Practitioners and the acute hospital, as well as with therapists within the Integrated Health Teams.

The Integrated Community Health Service operates a seven day service from 08:30 - 22:00, so flexible working is required. Therapy team will be rostered between 08:30 - 18:00 hours including weekends and bank holidays.The ability to travel independently across Warwickshire is required.
