Role summary
This fixed-term role for 10 months (or secondment) involves working as a Dementia Care Clinical Specialist within a supportive team. You will provide specialist assessment and care planning for inpatients with complex dementia needs, focusing on person-centered approaches. The role requires independent work, caseload prioritization, and collaboration with patients, families, and staff to optimize care and engagement. A thorough induction and regular supervision are provided.
Essential criteria
- Registered Nurse/ AHP on the appropriate part of the NMC/HCPC Register
- To have completed Mentor Preparation Programme or demonstrate mentorship experience if applicable to clinical area
- Post Registration qualification relevant to the speciality
- Able to perform clinical skills relevant to the speciality
- Attitude, aptitude

- Demonstrates understanding and commitment to Equality, Diversity and Inclusion

Job summary
Fixed term contract/ secondment opportunity to 26 April 2027.
We are seeking a person who enjoys working with people living with dementia and who values the importance of person-centered care, for a fixed term contract for 10 months. Secondments may be available for internal candidates with the agreement of your manager.
The Dementia Support Service is a well-respected, supportive and hardworking team of support workers, nurses and occupational therapists who are dedicated to optimising the care of in-patients with dementia and enabling best outcomes for them at the point of discharge.
As a dementia specialist practitioner your focus will be on providing specialist assessment and care planning for patients who present with complex needs, along with interventions to enable engagement in care. You will promote person centered approaches, working collaboratively with patients, their families/carers and staff.
