Role summary
This role is for a registered nurse to join the Bognor Regis Central PCN Community Team. It involves providing diverse community nursing care, including diabetic management, wound care, palliative care, and bladder and bowel care. The successful applicant will work with local health and social providers, focusing on rehabilitation, self-management, and patient independence. An individual induction plan will be provided for new team members.
Key responsibilities
- Manage diabetic patients, including education and treatment.
- Provide comprehensive wound management, including leg ulcer diagnosis and treatment.
- Deliver palliative and end-of-life care to patients in the community.
- Manage bladder and bowel care, including catheterisation and continence needs.
- Work in partnership with local health and social providers, particularly for frailty patients.
- Support evidence-based care and facilitate staff learning and development within the team.
Essential criteria
- RN qualification / degree or diploma
- ENB 998 or equivalent or work towards
- Good numeracy and literacy skills
- Good communication and negotiation skills
- Is able to demonstrate the application of research based practice
- Knowledge of current local policies informing community health and social care services
- A clear understanding of the role of the Community Staff Nurse
- Promote independence and choice in undertaking patient care
- IT literate or willing to achieve within an agreed period
- Knowledge of Clinical Governance, clinical audit and clinical supervision, Professional codes of Conduct Ability to demonstrate knowledge/ understanding of boundaries of role Practice supervisor
