Job summary
We are delighted to offer an exciting opportunity to join our team as a Neonatal Outreach Nurse at Mid Yorkshire Teaching NHS Trust. This is a fixed term post - 2 years contract.
The primary aim of the role is to ensure that a high-quality Neonatal out reach care is delivered to neonates and their families.
The Nurse will plan, deliver, and evaluate the care of Neonate and their families in the neonatal unit and home setting.
As a Nurse, all activities provide opportunities for acting as a role model for staff.
The post holder will be responsible for ensuring the planning, delivery and evaluation of care to meet the personalised health and wellbeing care needs of patients, in line with the Trusts Values and Nursing & Midwifery Professional Framework
This will be achieved through the core competencies/ dimensions of the role, clinical practice, and acting as the patient advocate.
Main duties of the job
- To assist staff, based on the Neonatal Unit and Transitional Care Unit, with early and appropriate discharge planning in accordance with the criteria for neonatal outreach care.
- To provide a seamless timely discharge from the hospital to the home environment.
- To provide on-going holistic care within the community setting for vulnerable neonates
