Role summary
This role is for a motivated Registered Children's Nurse to join the Paediatric Outreach Team at Stoke Mandeville Hospital. You will deliver high-quality, family-centred care to children and young people in their homes and community settings, supporting those with acute, long-term, complex, and palliative care needs. The position involves working autonomously and within a multidisciplinary team, assessing, planning, delivering, and reviewing individualised care. You will also provide specialist nursing interventions and education to families and carers.
Key responsibilities
- Work autonomously and within a multidisciplinary team, using sound clinical judgement in complex situations.
- Assess, plan, deliver and review individualised care with children, young people and families.
- Provide specialist nursing care through home visits, clinics and telephone support.
- Deliver clinical interventions including intravenous therapy, vascular access care, enteral feeding management, oxygen therapy, suctioning, wound care and palliative care.
- Recognise and respond to patient deterioration and escalate concerns appropriately.
- Support timely discharge, preventing avoidable admissions and ensuring seamless care between hospital and community services.
Essential criteria
- Highly skilled clinical practitioner with experience within acute and community settings.
- Ability to communicate clearly with patients, relatives and MDT team in English
- A comprehensive understanding of professional nursing issues
- Ability to work under pressure/meet tight deadlines whilst delivering high quality care
- High motivation and ability to motivate others
- Computer literate
- Good time management skills
- Managerial and leadership ability
- Ability to resolve conflict/ dispute in a non-confrontational manner
- Registered Children's Nurse
- Teaching/Assessing Certificate or equivalent
- Evidence of further continued professional development
- Safeguarding Level 3 Training
- Extensive and recent experience in a variety of Paediatric settings
- Experience of managing and leading a team as a Band 6 Senior Nurse
- Experience of Quality management against current National Paediatric Standards/ Children and Young People
- Active participation and experience of clinical governance processes
- Experience of change management and exceptional service delivery
Desirable criteria
- Flexibility and ability to cope with different work environments
- Degree level qualification

We are seeking a motivated Registered Children's Nurse to join the Paediatric Outreach Team based at Stoke Mandeville Hospital.
This role involves delivering high-quality, family-centred care to children and young people in their homes and community settings. The service supports children with acute and long-term conditions, complex healthcare needs and palliative care requirements, including end of life care.
Working within a multidisciplinary team, you will:
Deliver nursing care in home and community settings.
Support children and young people with acute, chronic and complex health needs.
Coordinate care for children with complex conditions.
Provide palliative and end of life care when required.
Work with families, carers, schools, primary care services and healthcare professionals.
Assess, plan, deliver and evaluate care.
The team works closely with the acute paediatric ward, providing seamless care across hospital and community settings.
