Role summary
This role is for an experienced Band 7 Community Neurology Specialist Nurse with a focus on Parkinson's Disease. The post holder will develop and implement a seamless community PD specialist service, ensuring high standards of clinical care and providing specialist advice and education. They will work autonomously to assess, manage, plan, and implement treatment tailored to individual patients across the Parkinson's disease spectrum. The role involves collaborating with Primary Care Networks and other services to meet local population needs.
Key responsibilities
- Function autonomously to assess, manage, plan and implement treatment tailored to individual patients across the Parkinson's disease spectrum.
- Employ a collaborative approach with the wider multidisciplinary team to ensure care for patients and their families is effectively coordinated.
- Continually develop and update knowledge and highly specialist skills to maintain specialist competence and ensure delivery of evidence based care.
- Maintain the Parkinson's Disease patient caseload including acceptance of appropriate referrals, triage, waiting times and discharging patients.
- Develop, deliver and evaluate a plan of care relating to symptom management, including assessment, planning, implementing, reviewing and documenting care plans.
- Design and deliver education and self-management courses to people with PD/MS and their families, and provide specialist education to other professionals.
Essential criteria
- Registered Qualification in Nursing
- Relevant post registration training in Parkinson's Disease or willingness to undertake
- Non-medical Prescriber or willingness to undertake within the first year in post
- Advance skills in Clinical Assessment or willingness to undertake within the first year in post
- At least 3 years post registration experience working at Band 6
- Understanding of Parkinson's Disease and current treatment options
- Understanding of the NHS Long Term Plan
Desirable criteria
- Ability to prepare and deliver presentations to groups of people

We are looking to recruit an experienced senior nurse to work as a Band 7 CommunityNeurology Specialist Nurse with interest in Parkinson's Disease.
We welcome applications from experienced or existing Band 7 Parkison's Disease SpecialistNurses as well as nurses working at Band 6 level aspiring to progress into aspecialist nursing role within neurology.
The post holder will develop, implement and evaluate a seamless community PD specialist service ensuring that patients receive the highest standard of clinical care and that specialist advice, education and support to health professionals and patients is provided.
They will work collaboratively with the established Primary Care Networks, ensuring cohesive leadership across the three localities.
The post holder will be an integral part of the West Essex Care Closer to Home model, working in partnership with Social Services and other statutory and non-statutory services to meet the needs of the local population using public health information/population management to support service development.
