Key responsibilities
- Proactively manage patients living with frailty and other long-term conditions in conjunction with PCNs.
- Lead and facilitate patient-centred coordinated case management across primary, community, and secondary care.
- Work autonomously to a high standard of clinical care, identifying patients who would benefit from intervention.
- Assess, diagnose, and prescribe for acute medical conditions using a holistic approach.
- Design multi-disciplinary patient pathways following comprehensive geriatric assessment to support patient choice and promote self-management.
Essential criteria
- Educated to degree level in chosen profession.
- * BSc Nursing or RN currently registered with NMC.
- * BSc Occupational Therapy
- * BSc Physiotherapy
- Post registration degree or master's degree
- History taking and physical assessment course
- Independent and supplementary prescribing course or is willing to undertake within first year in post if applicable.

Job summary
Join a new innovative community health team delivering proactive, person-centred care across Southampton neighbourhoods. As a Neighbourhood Care Practitioner, you will work at the intersection of primary care, social support, and community services to prevent hospital admissions, support independent living, and improve wellbeing for people in their homes and local settings. This is a hands-on, autonomous role with strong clinical collaboration and opportunities to shape service design.
Posts available: Three full-time posts, one in each of the three localities across Southampton with cross cover when applicable.
Contract: Fixed 12-month contract with the possibility of extension following evaluation.
Hours are full-time, flexible working patterns considered.
