Role summary
This is an exciting opportunity for a kind and experienced Senior Community Practitioner to join the Whitby and Ryedale Integrated Community Team at The Anchorage Clinic. The role involves managing a caseload, delivering care autonomously, participating in the duty worker rota, and supporting junior team members and students. The team provides mental health services to adults in the Whitby area, working collaboratively with a multi-disciplinary team.
Key responsibilities
- Hold a case load of patients as a named worker/care coordinator.
- Deliver care to meet identified patient needs autonomously and with MDT support.
- Participate in the duty worker rota and support the patient journey from referral to discharge.
- Provide support to junior members of the team.
- Provide an excellent learning environment for students placed with the team.
Essential criteria
- Current professional registration with an approved professional body.
- Evidence of Continuing Professional Development.
- For registered nurses: MIP, FLIP, ENB 998 Teaching & Assessing in Clinical Practice or City & Guilds 730 Teaching in Adult Education. Must be achieved within agreed timescale
- 18 months post qualifying experience including sign off of appropriate preceptorship or competency framework.
- Recognised sign-off mentor, clinical educator or equivalent. Must be achieved within agreed timescale
- Key skills in literacy, numeracy and ITQ level 2 (or equivalent)
- Significant experience working with people with mental ill health in a care environment.
- Working collaboratively with service users and their families/carers.
- Providing clinical supervision to individuals or groups as professionally appropriate.
- Mentoring or facilitating students on practice placement. Working in a multi-disciplinary team
- Clinical Risk Assessment and Management and its application in practice.
- Understanding of psychological models of care and treatment
- Understanding of relevant legislation (e.g. Mental Health Act, Mental Capacity Act).
- Communicate complex and sensitive information effectively to patients, carers/families and all members of the multidisciplinary team.
- Work effectively as part of a multidisciplinary team and undertake lead professional / care co-ordinator responsibilities
- Provide effective clinical supervision, teaching, training and assessing in clinical practice
Desirable criteria
- Post-graduate qualification in a relevant area
- Leadership or management qualification
- Significant experience working with mental ill health in a community environment.
- The Trust's Quality Improvement System (QIS)
- Competency in venepuncture

An exciting opportunity has arisen for a kind and experienced Senior Community Practitioner (part time - 30 hours) to join our friendly and dedicated colleagues at The Anchorage Clinic in Whitby.
Whitby and Ryedale Integrated Community Team provides mental health services and delivers care to adults across the Whitby area.
The Anchorage is a short walk from the main town and harbour where there are many local shops and cafes. Whitby is known for its unique blend of rich history, beauty and charming tranquil lifestyle.
As a member of the team, you will work alongside all members of the multi-disciplinary team which includes Registered Nurses, Social Workers, Psychologists and Psychology Assistants, Consultant Psychiatrists and medics, Pharmacists, Team admin and support workers. With access to and support from specialist services available in the trust, and excellent working relationships with local partner organisations.
