Role summary

This role is for a dynamic and flexible Rehabilitation Assistant within the Early Intervention Community Team at Kenrick Centre. The team focuses on preventing hospital admissions and facilitating discharges. You will work alongside Nurses and Therapists to provide innovative care to adults in the community, contributing to comprehensive care plans. The position requires working shifts to provide a seven-day service.

Essential criteria

  • NVQ level 3 in Health and Social Care or equivalent experience or evidence of completion of in house further opportunities programme
  • English and Maths GCSE/Good standard of education
  • Relevant experience in delivering direct patient care in a health or social care setting.

Desirable criteria

  • Experience of working in a rehabilitation setting.
  • Skills/Knowledge
  • Effective oral and written communication skills
  • Basic IT Skills
  • Able to work in a team
  • Able to work independently
  • Awareness of the needs of a multicultural, socially diverse population.
  • Awareness of responsibilities related to Safeguarding Vulnerable adults and children

Main duties of the job

We are currently looking to recruit a dynamic, enthusiastic and flexible individual that will perform a pivotal role within the Early Intervention Community Team, giving you the opportunity to show how you can provide an innovative service to adults within the community as well as contributing to a comprehensive care plan for the individual service user.

The Early Intervention Community Team is focused on preventing avoidable hospital admission and facilitating discharge. You will have the opportunity to work alongside Nurses as well as Therapists in meeting these goals.
