Role summary
This is an exciting opportunity for an experienced Band 7 Clinical Team Lead to join the newly formed Hounslow Intensive Outreach & Rehabilitation Team (HIORT). You will provide visible clinical leadership, manage staff, support caseloads, and help shape the integrated multidisciplinary team. The role involves hands-on clinical leadership, retaining a small caseload, and working alongside practitioners to strengthen clinical standards and improve service user experience. You will be a registered mental health professional with a strong background in community mental health.
Essential criteria
- Registered Mental Health Nurse, Occupational Therapist or Social Worker, with current live registration with the NMC, HCPC or Social Work England.
- Post registration training and continuing professional development relevant to psychosis, rehabilitation or assertive outreach.
- Training in supervision, mentorship or practice education.
Desirable criteria
- Training in psychosocial interventions or Thorn.
- Family therapy or family work training.
- Training in substance misuse or dual diagnosis.
- Degree or postgraduate qualification relevant to the care group.
- Evidence of leadership or management training, or a willingness to undertake this.
- Knowledge & Skills
- Detailed understanding of psychosis, rehabilitation and assertive outreach
- approaches and their impact on service users and carers.
- Knowledge of the Mental Health Act, Mental Capacity Act and other legislation relevant to this client group
- Skills in risk assessment and risk management, including in situations of high clinical complexity
- Skills in engaging and sustaining contact with people who are reluctant or difficult to engage.
- Understanding of the needs of diverse communities and skills in engaging with people from a range of backgrounds.
- Excellent verbal and written communication skills, including report writing, presentation and IT skills.
- Ability to work autonomously, prioritise competing demands and make decisions under pressure.
- Skills in psychosocial interventions. Knowledge of dual diagnosis models and of working alongside substance misuse services.
- Post registration experience at Band 6 or equivalent in a community mental health setting, working with people with severe and enduring mental illness.
- Experience of holding and managing a complex caseload, including assessment, care planning, risk management and review.
- Experience of providing clinical and management supervision to qualified staff.
- Experience of undertaking appraisals or Personal Development Reviews.
- Experience of day-to-day staffing management, including rostering, annual leave and sickness absence
- Experience of incident reporting and review, and of contributing to investigations,
- complaints and safeguarding processes.
- Experience of maintaining clinical records on an electronic patient record system, and of monitoring record keeping standards within a team.
- Experience of working within a rehabilitation or assertive outreach service.
- Experience of deputising for a manager, or of holding day to day operational responsibility for a team.
- Experience of Healthroster or a comparable rostering system.
- Experience of RiO or SystmOne.
- Experience of leading or contributing to service change and quality improvement.
- Experience of working with supported accommodation and housing providers.
- Experience of working with people who use substances.
- Experience of audit, research or teaching.

Job summary
An exciting opportunity to help shape a new specialist community mental health service in Hounslow.
We are looking for an experienced and enthusiastic Band 7 Clinical Team Lead to join our newly formed Hounslow Intensive Outreach & Rehabilitation Team (HIORT).
HIORT brings together our established Community Rehabilitation (CRS) and Assertive Outreach (AO) pathways into one integrated multidisciplinary team. This is an important development for Hounslow and an opportunity to build on the strengths, expertise and experience of both services, while retaining the distinct clinical approach each pathway needs and creating a more joined up offer for people with complex and enduring mental health needs.
As our Clinical Team Lead, you will be right at the heart of this development.
Working alongside the Band 8a Team Manager, and our wider multidisciplinary colleagues, you will help bring the new team together and shape how we work in practice. You will provide visible, supportive clinical leadership across both pathways, line manage and develop staff, support effective caseload management and clinical decision making, and help us establish a positive, accountable and compassionate team culture.
