Role summary
This role is for an experienced Band 6 GP Plus Practitioner to join the Temporary Staffing Team in Stevenage. You will work across primary care and mental health services, facilitating mental health assessments and supporting GPs with referral decisions. The post holder will also help transition service users from secondary to primary care and provide consultation and education to local primary care teams. The position involves working with adults of working age with mental health problems.
Key responsibilities
- Undertake assessments of individuals and their families within primary care, including risk assessment.
- Be a core member of primary care teams, using specialist mental health knowledge.
- Develop links and foster partnership between Primary Care Teams and HPFT Adult Mental Health Services.
- Promote the use of treatment protocols and care pathways.
- Enable workers in Primary Care to develop skills and confidence in mental health assessment and treatment.
- Facilitate joint work between Primary Care and Secondary Care interface and Primary Care/Voluntary Sector.

Job summary
We are currently seeking an experienced GP Plus Practitioner, at Band 6 to work via a Bank Contract, based in Stevenage for our Primary Care Mental Health Service. You will be joining our Temporary Staffing Team within HPFT - Hertfordshire Partnership University NHS Foundation Trust.
The role of the GP Plus Practitioner will be to work across the interface between Primary Care and Mental Health Services to work alongside the service manager(s) to facilitate mental health assessment in primary care and support GPs about decision making in relation to secondary care referrals, support transition of service users from the care of secondary mental health services back to Primary Care and to facilitate re-access where appropriate.
The post holder will do this by providing consultation, support, education and problem-solving approaches to the Local Primary Care Teams particularly in the area of assessment and management of mental health problems in adults of working age.
