Job summary

The Crisis Resolution Home Treatment Team in Southampton are truly multidisciplinary with practitioners from a variety of disciplines and backgrounds.

As a team, we support individuals with acute mental health needs requiring intensive input to remain in the community. The team also works collaboratively with our colleagues on the wards at Antelope House to help reduce the amount of time patients remain in hospital by offering support via hospital at home.

Our service provides a variety or clinical interventions to collaboratively tailor our care plans to each individual service user. These include psychological interventions, medication support, prescribing, access to group work and cares support.

This can be offered as either an internal staff secondment for current employees of HIOWH NHS FT or as a fixed term contract for external trust staff

As a Senior Practitioner in the team you will have a key function in assessing service users as they present in crisis and require an urgent response.
