This contract type is not eligible for Skilled Worker sponsorship.
NHS Trusts cannot issue a Certificate of Sponsorship for part-time, bank, locum, zero-hours, job-share or short fixed-term (under 12 months) contracts under current Home Office rules. This applies even when the wider Trust holds a Skilled Worker licence.
Role summary
This role involves empowering people to take control of their health and well-being by connecting them to practical and emotional support in the community. You will provide non-clinical support to vulnerable residents, focusing on prevention and early intervention. The position is practice-based, not remote, and requires working collaboratively with local partners to strengthen community resilience and reduce health inequalities. You will support residents to become more independent and enhance their well-being.
Essential criteria
- Experience of working directly in a community development context, health and social care, learning support, public health or health improvement. This can include unpaid work.
- Experience in supporting people, their families and carers in a related role.
- Experience in support planning
- Experience working with the VCS sector (in a paid or unpaid capacity), including with volunteers and small community groups
- NVQ Level 3 Advanced level or equivalent qualifications or experience, or working towards. Supporting evidence will be required if you are working towards a level 3 NVQ.

- Previous experience of working within a PCN.

Job summary
The London
Borough of Waltham Forest and Waltham Forest Federated GP Network are
supporting the Primary Care Networks established across the borough
to recruit Social Prescribing Link Workers.
The post will be part of a wider Integrated Care development known as the Managed
Network of Care & Support.
The role is practice based and not remote.
Salary: Agenda for change - Band 5 - Pro rata
Hours: 18 hours 45 mins. Part time
Fixed Term Contract: Between 6-8 months. To be discussed during interview.
Probation period: 3 months (Extension/s can be given as per our policy and only if required).
The Managed Network focuses on prevention and
early intervention. A key activity is to
provide non-clinical support to vulnerable residents by employing link workers
across 3 well-being pathways, Social Prescribing, well-being at Home and Local
Area Coordination.
The model has been
developed as part of the Waltham Forest Integrated Care Partnership and is part
of the Promoting Wellbeing Programme.
