Role summary
This role is for a Specialist Community Public Health Nurse (Health Visitor) within an integrated skill-mixed team, delivering the 0-19 Universal Health Service in Blackburn with Darwen. The post holder will lead on the delivery of the Health Child Programme 0-5 years, providing family-centred, evidence-based public health services. The aim is to proactively improve health outcomes for a defined caseload, population, or community, reducing health inequalities by working in partnership with families, communities, and other professionals. Safeguarding is a key component, ensuring safe and effective management of risk for children, young people, and families.
Key responsibilities
- Works as the named Health Visitor for a defined caseload of antenatal mothers, children and families ensuring full responsibility for their case management.
- Plans, delivers and evaluates an innovative service using evidence based knowledge and works effectively with key agencies.
- Provides a holistic assessment of the needs of families taking into account physical, psychological, social and environmental factors and develops action plans.
- Delivers targeted intensive interventions to the most vulnerable families building therapeutic relationships and addressing difficult issues.
- Presents a positive image of the service when meeting service users and partners.
- Demonstrates advanced communication skills to deal with complex and volatile situations and participate in decision making.

- Degree level qualification V100/V300 prescribing

- Child and adult safeguarding knowledge/experience
- IT Skills
- Access to a vehicle for work - Able to comply with the travel requirements of the posts
