Role summary
This role is for a registered nurse with skills in palliative/end of life care, wound care, catheter care, and urgent community 2-hour response. You will work within the Urgent Community Response (UCR) Team to prevent unnecessary hospital admissions and support early discharge. The position requires working a 7-day week, including weekends, and involves holistic assessment and care pathway prescription for patients in crisis. You will also support junior staff and students.
Key responsibilities
- Work within the Urgent Community Response Team to prevent unnecessary admission to hospital.
- Assess and prescribe care pathways for patients in a crisis situation with long term conditions, frail & elderly, palliative and end of life care, and/or rehabilitation needs.
- Act as the patient's advocate, facilitating choice and patient empowerment.
- Provide evidence based clinical/therapeutic interventions based on "best practice".
- Work within the Urgent Community Response Team to support early discharge from hospital.
- Triage referrals on a rotational basis for the Urgent Community Response Team.
Essential criteria
- Diploma or 1st Level Degree
- RN Qualification
- Current NMC Registration
- Evidence of Continuing Professional Development
- Evidence of working within the community or acute setting
- Attendance at in-house training and short courses
- Good communication skills

- The post holder will need to be passed fit to perform full duties of the post through occupational health with any reasonable aids and adaptations if necessary.
- Must hold full and valid and driving licence and have access to a vehicle
- Able to communicate effectively in written and verbal English Language
- This is a 7 day a week service and some weekend working will be expected. Hours of service delivery, 8am-8pm, 365 days per year

Job summary
Opportunity for a registered nurse with skills in palliative / end of life care, wound care, catheter care and urgent community 2 hour response for patients and a strong desire to further develop your community nursing skills and experience.
You will be required to work over a 7 day working week including weekends and will be hard working, motivated, with excellent communication skills. You will be able to demonstrate knowledge and skills in holistic assessment, to demonstrate that you use your initiative and have a keen interest in working with this group of patients. You must hold a full and valid driving licence and have access to a vehicle.
You will support staff working at a band 3 and 4 level, along with students and apprentices.
Due to the unplanned nature of the role we would recommend newly qualified staff to not apply.
