Role summary
This role is for a Senior Community Mental Health Nurse within the City CMHTs in central Norwich. The post holder will manage a caseload, provide direct patient care, and conduct risk assessments within a multidisciplinary team. They will also coordinate care, attend patient reviews, and supervise junior colleagues and students. The team focuses on recovery and provides a supportive environment for service users and carers.
Key responsibilities
- Manage a caseload and provide mental health care services.
- Provide planned care, risk assessment, and screening for patients, evaluating needs and identifying suitability for discharge.
- Initiate and attend patient-centred reviews, case conferences, and other meetings.
- Advise the Responsible Clinician(s) and other professionals of changes in patients' conditions.
- Work within the MDT and form partnerships with other care providers.
- Participate in the duty worker rota (Monday to Friday 09:00am to 17:00pm).
Essential criteria
- Registered Mental Health Nurse with the relevant professional registration
- Nursing degree or equivalent Mentoring / Mentorship (ENB988 or equivalent)
- Ability to travel independently
- Be able and willing to undertake further training / development specific to the role.
- Experience of working within a multidisciplinary team
- Familiar with risk assessment and its management
- Able to demonstrate the ability to process, plan and delegate work
- Ability to identify and action change in health care need
- Satisfactory verbal and written English language skills
- Up to date and evidence-based knowledge of Person Centred Care
- Working knowledge / application of the Mental Health Act
Desirable criteria
- Evidence at social care / health related study
- Ability to transport patients as appropriate
- Able to liaise with statutory / non- statutory bodies due to patients' needs.
- Previous community experience
- Able to time manage and reflect on clinical practice
- Knowledge of mental health needs for older people.
- Knowledge of national policy influencing older persons' health care

The team is based in central Norwich and covers central based Gp practices.
