Role summary

Wellspring [contact details removed] is seeking a part-time Clinical Pharmacist to manage long-term conditions and support patient care. The role involves working independently within a multi-disciplinary team, conducting medication reviews, and providing clinical leadership on medicines optimisation. The surgery will support the successful candidate in achieving the CPPE pathway if not already completed. This position aims to enhance the general practice workforce and improve patient outcomes.

Job summary

Wellspring

[contact details removed] is recruiting a part-time (15-20 hours) Clinical Pharmacist who has a keen interest in

providing long term conditions management with our patients. You will be able

to practice independently and be a valuable part of the team. Applications are

welcome from Clinical Pharmacists both with and without the Independent

Prescribing qualification.

The purpose

of the role is to support the general practice workforce and improve patient

care. We are keen that this role develops in line with the national

Clinical Pharmacist programme. Ideally the postholder will have

completed the CPPE pathway, but the surgery will support the right candidate to

achieve this: Primary care pharmacy education pathway

: CPPE.

Main duties of the job

Working as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas

Supporting patients with chronic disease management

Undertaking clinical medication reviews

Managing prescription processes and medication queries

Providing clinical leadership on medicines optimisation and quality improvement, including QOF

Development support and mentorship will be provided on a practice level and across the PCN

You will be working alongside other Pharmacists and a Pharmacy Technician
