Role summary
This is an exciting opportunity for a motivated Band 6 Occupational Therapist to join a friendly and supportive Intermediate Care Team. The role is peripatetic, working across inpatient intermediate care units to support patients on their P2 journey using the 'Discharge to Assess Model' and 'Home First' approach. The successful candidate will undertake essential assessments and OT interventions to maximise independence and facilitate discharge. This position is 30 hours per week on a permanent contract, based at Perry Tree Centre.
Essential criteria
- Occupational Therapy Degree
- HCPC registration
Desirable criteria
- Student educator certificate
- Significant post qualification experience of having worked in an Inpatients setting working with a variety of medical conditions
- Experience working with patients with complex diagnosis/ conditions
- Experience of complex Discharge planning
- Experience of working in a Multi-Disciplinary team
- Management experience
- Knowledge and Skills
- Knowledge of a range of appropriate assessment and treatment approaches for the client group and evidence of the ability to manage complex patients
- Demonstrates a problem solving approach
- Ability to act as mentor/supervisor to junior staff and students on placement
- Ability to safely handle patients and equipment
- Demonstrate an understanding of the Discharge to Assess model
- IT skills in Microsoft, PowerPoint, Excel and Outlook
- Demonstrates clear and effective verbal and communicative skills
- Demonstrates awareness of and sensitivity to the needs of individuals from diverse social and cultural groups
- Able to work independently and delegate work to others appropriately
- Demonstrates a committed and organised approach to CPD

An exciting opportunity has arisen for a motivated and enthusiastic Band 6 Occupational Therapist to join our friendly and supportive Intermediate Care Team.
The post is 30 hours per week on a permanent contract.
This peripatetic role offers the opportunity to work across our inpatient intermediate care units, supporting patients on their P2 journey. We work to the 'Discharge to Assess Model' using the 'Home First' approach, with the emphasis on carrying out essential assessments and OT interventions to maximise independence and to facilitate discharge. Working from Perry Tree Centre (PTC), you will be part of a dynamic multidisciplinary team committed to delivering high-quality, patient-centred care.
We are looking for a passionate and motivated OT with previous In-Patient experience who is committed to continuing to develop the OT role on our In-Patient wards, taking into account the current and changing climate of the NHS. Whether you are an experienced Band 6 or a Band 5 looking for the next step in your career, we would love to hear from you.
As this post involves carrying out regular Access Visits to patient's homes, the successful applicant will ideally need to be a car driver.
