Role summary
This role involves supporting vulnerable individuals who have entered the criminal justice system, particularly those released from police custody with additional health needs. The focus is on providing up to 12 weeks of community support to reduce reoffending risks and address health vulnerabilities. Duties include helping people engage with health appointments, mental health services, and drug and alcohol support, as well as providing psycho-social advice.
Key responsibilities
- Independently undertake face to face and electronic screening to support the identification of multiple vulnerabilities of people who have been in contact with the criminal justice system or immigration centre.
- Work within limitations and escalate to qualified staff as appropriate.
- Provide an embedded service in police custody suites, courts and prisons working within legislation requirements, such as PACE (1984).
- Work as part of a team with the freedom to work independently, particularly in relation to providing advice to service users, carers, and professionals.
- Work with service users in conjunction with other professionals, to devise and implement effective individualised programmes of support.
- Liaise with other agencies as required dependant on the persons presentation, work closely with other agencies, such Custody and Prison Health Care Professionals, and community agencies.
Essential criteria
- NVQ Level 3 Health & Social Care (or willingness to work towards) or equivalent experience.
Desirable criteria
- The post holder will require additional vetting by partner agencies; this includes Police Vetting and / or Prison Clearance.
- Experience of working with a range of vulnerabilities which may include mental health, physical health, learning disabilities, abuse, addiction and social issues.
- Experience of working with a range of statutory, non-statutory agencies.
- Experience and/or sound knowledge of the Criminal Justice System.
- Experience of working in a community based setting.

The role of the band 4 outreach support worker is to support vulnerable people who have entered the criminal justice system. The service has a focus on supporting adults who have been released from police custody and may have additional health needs. Main duties of the job
The main focus of the outreach service is to support vulnerable adults to engage in the services available in the community. This may mean supporting people to health appointments, engagement in mental health and drug and alcohol services. Support with accessing online therapeutic interventions that will be available. Providing psycho social approaches, advice and guidance to help support health inequalities and support approaches to help reduce the risks of reoffending.
