Role summary
This role is for an Integrated Offender Health Navigator to support individuals who have been in contact with the criminal justice system. The navigator will screen for multiple vulnerabilities, assist in navigating health and social care services, and make onward referrals. They will work within a multi-disciplinary team, providing advice and support to service users and professionals. The role involves working in police custody suites, courts, and outreach settings, and completing court reports.
Key responsibilities
- Independently undertake face to face and electronic screening to support the identification of multiple vulnerabilities of people who have been in contact with the criminal justice system.
- Work within limitations and escalate to Practitioner as appropriate.
- Provide an embedded service in police custody suites, courts and Outreach, working within legislation requirements, such as PACE (1984).
- Work with service users in conjunction with other professionals, to devise and implement effective individualised programmes of support.
- Liaise with other agencies as required dependant on the person's presentation, work closely with other agencies, such Custody and Health Care Professionals, and community agencies.
- Complete Court Reports outlining individuals needs that may need be taken into account when decisions about charging and sentencing are made.
Essential criteria
- NVQ Level 3 Health & Social Care (or willingness to work towards) or equivalent experience.
- Experience of working with a range of vulnerabilities which may include mental health, physical health, learning disabilities, abuse, addiction and social issues
- Experience of working with a range of statutory, non-statutory agencies
Desirable criteria
- Experience of working in a community based setting
- Experience and / or sound knowledge of the Criminal Justice System
- Experience of implementing personalised care plans or rehabilitation
- Experience of individual and group work
